A student's IEP or 504 changes mid semester, and the testing plan never catches up. The issue may not be caught until the test has already started, and by then it may be too late to correct it. Many states are moving to three-times-a-year testing, which means three separate chances for an accommodation to be missed. Transfer students and new coordinators add to the complexity.
